Preterm delivery can result from premature rupture of the membranes, multiple gestation, incompetent cervix, fetal malforma-tion, fibroid uterus, uterine malformation, severe preeclampsia, abruptio placentae, or a wide variety of other medical and obstetric complications. Studies suggest that preterm labor is often preceded by upper genital tract infection. Thus, the etiology of preterm labor often differs from that of labor at term. The term “spontaneous preterm labor” has often been applied when no predisposing factor has been identified. Perhaps “idiopathic preterm labor” would be a more accurate description. The influence of preceding abnormalities on the fetus who delivers prematurely is apparent from ultrasound estimates of fetal weight. For a given gestational age, the estimated weights of those with subsequent preterm delivery are significantly lower than those who ultimately deliver at term. Thus, preterm delivery is an obstetric problem that does not occur in isolation. Optimal management requires an awareness of the
