中心点目的:评价Stevens-Johnson综合征(SJS)患者的功能视力(FVA)与眼表表现的相关性。中心点设计:前瞻性比较研究。中心点方法:对在东京医科大学东京牙科大学和日本京都地方医科大学眼科就诊的38例慢性SJS患者的69只眼进行研究。对照组10例20眼,干燥综合征(SS)患者20例40眼。常规行Landolt视力(VA)、FVA检查及裂隙、灯检查。用FVA测量系统在30秒内连续测量一只眼的FVA。视力维持比(VMR)计算如下:VMR=[(2.7-FVA)/(2-7基线VA)],其中FVA最小分辨角度的对数被输入公式中,2.7代表该系列中最低的视力。结果:SJS组VMR明显低于SS组和正常对照组。FVA值与角膜混浊程度和血管化程度有关。结论:FVA测量系统不仅是评价SJS动态VA变化的有用工具,而且能反映SJS的眼表临床表现。
center dot PURPOSE: To evaluate the correlation of functional visual acuity (FVA) measurement with ocular surface findings in patients with Stevens-Johnson syndrome (SJS).center dot DESIGN: Prospective comparative study.center dot METHODS: Sixty-nine eyes of 38 patients with chronic SJS assessed at the Tokyo Dental College, Tokyo Medical Center, and the Kyoto Prefectural University of Medicine, Department of Ophthalmology, Kyoto, Japan, were studied. Twenty eyes of 10 normal subjects and 40 eyes of 20 patients with Sjogren syndrome (SS) were also studied. Conventional Landolt visual acuity (VA) and FVA examinations and slit,lamp examinations were performed. FVA was measured continuously by the FVA measurement system during a 30,second blink-free period in one eye. The visual maintenance ratio (VMR) was calculated as follows: VMR = [(2.7 - FVA)/(2-7 baseline VA)], where logarithm of minimal angle of resolution values of FVA were entered into the formula and 2.7 represented the lowest visual acuity in this series. Slit,lamp examinations, Schirmer test, and fluorescein vital stainings were also performed in all subjects.center dot RESULTS: VMR was markedly lower in patients with SJS compared with patients with SS and controls. FVA values showed a relation with the presence of corneal opacity and vascularization.center dot CONCLUSIONS: The FVA measurement system is not only a useful tool in the evaluation of dynamic VA changes, but also reflects the ocular surface clinical findings in SJS.
