In 2017, the World Health Organization (WHO) defined grade 3 (G3) pancreatic neuroendocrine tumor as well-differentiated with a Ki-67 labeling index, typically between 20 and 55%. 1 G3 gastroenteropancreatic neuroendocrine neoplasms (GEP-NENs) were then classified as neuroendocrine tumors (NETs) and neuroendocrine carcinomas (NECs) under the revised classification scheme of WHO in 2019. 2 Nevertheless, the revised systems did not specify the upper cut-off of Ki-67 for G3 gastric NETs (g-NETs) and G3 intestinal NETs. The present study, therefore, aimed to explore an upper cut-off of Ki-67 for the concept of G3 NETs and G3 NECs in gastrointestinal (GI) NENs (GI-NENs) and to check the validity of classification (2019) for the predictive prognosis of G3 GI-NENs patients.
