Late segmental vein graft occlusion [LSVGO] in man and animals may be due to fibrosed venous valves. A segment of jugular vein containing at least 1 valve was reversed and autogenously grafted to the carotid artery in 17 dogs (25 vein grafts). After various lengths of time the grafts were removed and studied by light microscopy. Subendothelial proliferation was seen in all grafts after 2 wk. In 8 of the 25 grafts valve leaflets were destroyed and in the other grafts the valve leaflets were thin, pliable and mobile. In 4 of the 25 grafts a diaphragm-like structure developed distal to the valve and occluded 20-40% of the vessel lumen. Valvular fibrosis may not be the etiology of LSVGO but the normal valve leaflet may contribute to the formation of a diaphragm-like structure. Selection of valveless venous segments or valvulectomy of graft sections may reduce the occurrence of LSVGO.
