The results of the study by Madsen et al (10) highlight the challenges of studying a heterogeneous disease such as sepsis and the inherent difficulties of extrapolating biomarkers or treatments from one population to another. This study provides important clues in our ongoing quest for agents/markers involved in immunomodulation in critically ill patients, and illustrates the point that a “one size fits all” approach would probably not work in heterogeneous conditions such as sepsis.
