An historical account is offered of the syndrome of delusional parasitosis. Two explanatory hypotheses developed at the turn of the century are identified and traced to current debates on the etiology of the condition. The clinical and therapeutic aspects of delusional parasitosis are touched upon and 4 cases with different etiological associations are reported. Apparently, organic factors must always be kept in mind as it is likely than in some cases pruritus and other tactile disorders (particularly in the elderly) may be elaborated into delusions of infestation.
