METHODRadiographs are made with each ankle placed lateral side down on a 14 x 17-inch Kodak RP film using cardboard holders without intensifying screens. To standardize the position of the foot, the patient is asked to push his heel down firmly into a right angle brace. A disassembled x-ray body thickness caliper can be used for this (Fig. 1). Tube to film distance is 40 inches. Table-top soft tissue technique is used. When radiographic conditions are proper, both anterior and posterior borders of the tendon are sharply outlined, particularly at the insertion of the tendon into the calcaneus. Irregular densities of the surrounding fat can obscure parts of the anterior border of the tendon in some cases, but with experience they are easily recognizable. The point of maximal thickening of the tendon is measured, and this varies in location when xanthomas are present because they are usually irregular. The radiograph of a 17-mm. xanthoma containing a small area of calcification is shown in Fig. 2. Calcifications this large have been seen in approximately one-quarter of the cases studied to date.Small skin calluses overlie the calcaneal insertion of the tendon in some individuals and extend for a few millimeters above it. These are not difficult to recognize and should not be included in measurements. Some obese patients have subcutaneous fat behind the posterior border of the tendon, usually this does not occur below the level of the tibia1 malleolus. Measurements of xanthomas are made where both anterior and posterior border of the tendon are sharply outlined. Several films with differing exposure factor> may be required to outline some xanthomas completely. If reasonable care is taken in positioning of the foot, duplicate tendon measurements should not differ more than hy I ml.(Table 1).
