METHODSThe participants in the study were men and women volunteers who recently had suffered an acute MI and agreed to participate in the pilot study that preceded the onset of the ENRICHD clinical trial. This pilot study was conducted from June through August 1996. 3 Myocardial infarction was determined by symptoms compatible with MI (eg, chest pain, nausea, vomiting, shortness of breath) and characteristic cardiac enzyme increases twice the upper limit for the institution in which the patient was hospitalized. Myocardial infarction also was determined by characteristic electrocardiographic ST segment change or new Q waves, or by enzyme or electrocardiographic evidence alone. The exclusion criteria specified the presence of a noncardiac condition likely to be fatal in 1 year, postcardiac procedural MI, severe mental illness, alcoholism, current enrollment in a research study investigating psychotherapeutic or pharmacologic treatment for depression or social isolation, patient refusal to provide informed consent, and physician refusal for the patient to participate.
