Objectives: Emerging evidence suggests that the neutrophil-to-lymphocyte ratio (NLR) may be a useful marker of inflammation and aortic stiffness. Markers of inflammation and aortic stiffness are both indicators of cardiovascular events. We, therefore, investigated whether the NLR is associated with aortic stiffness in patients with type 1 diabetes mellitus. Methods: We examined the relationship of the NLR to aortic stiffness in 76 people with type 1 diabetes and 36 healthy controls. Results: The NLRs in the group with type 1 diabetes were higher than in the controls (2.33 +/- 0.95 vs. 1.80 + 0.68, respectively; p = 0.003). Aortic strain and aortic distensibility, the parameters of aortic stiffness, measured noninvasively by the help of echocardiography, were significantly decreased in the patient group compared to controls (8.0% +/- 1.5% vs. 13.1% +/- 3.3 %; p
