S otalol (dl-sotalol), a class III agent, prolongs the ventricular action potential duration by blocking the rapidly activating component of the delayed rectifier potassium current.’The degree of lengthening of action potential duration by some class III drugs, including sotalol, has been shown to be attenuated at short ventricular paced cycle lengths (CL)(ie, reverse use dependence).* Schmitt et al3 recently used the monophasic action potential (MAP) to demonstrate the reverse use dependence of action potential duration with intravenous sotalol in the human ventricle. However, the prolongation of repolarization (QT interval) is reported to be more marked during chronic oral therapy with sotalol, despite lower serum levels of the drug compared with those after intravenous sotalol. 4 Therefore, this study examines the reverse use dependence of action potential duration by chronic oral sotalol in the human ventricle.
