FI G. 1. Line drawings illustrating the first American case report of concomitant pregnancy and endometriosis. The patient (case 14 in Sampson's series) was a 37 year old woman who compłained of intermittent dysmenorrhea, more during the previous year, and of menorrhagia which had increased over a 3-year period. She had been married for eleven years and had never been pregnant. On pelvic examination the uterus was felt to be irregularly enlarged and the cervix to be pushed forward by a hard tumor filling the cul-de-sac. The preoperative diagnosis of multiple leiomyomas of the uterus was confirmed at laparotomy. In addition, she was found to have a chocolate cyst of the left ovary which perforated during the course of dissection, numerous endometrial implants (imp.) on the posterior surface of the uterus, and an early uterine pregnancy. Microscopically the endometrial implants and the lining of the chocolate cyst exhibited a decidual reaction.(Left) The posterior aspect of the surgical specimen.(Right) Sagittal section of the same uterus, demonstrating an early pregnancy (14 mm. embryo). Perhaps the most succinct formulation of this view is that of Beecham (3):
