It is almost impossible to reverse iatrogenic gonadal damage after busulfan (BU)-based myeloablative pretreatment for haematopoietic stem cell transplantation (HSCT) without any risks under current conditions, although plenty of studies are devoted to fertility recovery (Fazeli et al. 2018). Feasible prophylactic measures to preserve ovarian function before HSCT and assisted reproductive technologies (ART) are conducive to salvaging fertility in survivors. However, the techniques of fertility preservation sometimes could not be achieved on account of a less developed medical condition or limited time before initial chemotherapy. Hormonal replacement therapy (HRT), if available and not contraindicated, can be offered as a satisfactory option for alleviating adverse effects of hypogonadism and regulating levels of oestrogen and progestogen. The restoration and maintaining of sex hormones serves as a catalyst for in vitro fertilisation (IVF) with donor eggs, which can be operated in such countries as China, where surrogacy is not allowed by legislation. Herein, we report a case that a young woman, who suffered from acute lymphoblastic leukaemia (ALL) and developed premature ovarian failure (POF) after myeloablative pre-treatment and HSCT, achieved successful childbirth through longterm HRT and ART with donor eggs.
