Traditional emplhasis on the extent of damage to the left ventricle in acute myocardial infarction has been properly related to the primary role of that chamber in the mechanical support of the circulation. Right ventricular infarction was initially a pathological observation until hemodynamic and radionuclide techniques afforded clinical recognition. A spectrum of clinical presentations on right ventricular infarction has been established from published reports, but the mana. gement of impaired right ventricular function in acute infarction of the left ventricle has awaited further clinical experience. l Initial observations in cardiogenic shock suggested that right ventricular involvement sometimes responded to volume overloading with subsequent enhancement of left ventricular filling pressure and cardiac output. In the January, 1981, issue of this JOURNAL, Grose and Spindola-Franc0 propose another clinical role for right ventricular dysfunction in extension of left ventricular infarction and rupture of the ventricular septum. 2 Thus, the pathophysiology, clinical recognition,, and hemodynamic significance of right ventricular infarction and function warrant further analysis.
