Follicular lymphoma is the most common indolent B cell lymphoma, accounting for 20% of all non-Hodgkin lymphomas1. Transformation of follicular lymphoma to a more aggressive lymphoma is a well-characterized phenomenon that occurs in 30% of cases at 10 years following diagnosis2.Follicular lymphoma most frequently transforms to diffuse large B cell lymphoma. Transformation to other neoplasms that retain expression of immunophenotypic markers of the B cell transcriptional program (Burkitt lymphoma, B lymphoblastic leukemia/lymphoma, classical Hodgkin lymphoma) is also well-described3–7. Follicular lymphoma can also transform into histiocytic/dendritic cell tumors, demonstrating that lineage infidelity is possible8–10. Rare reports of transformed follicular lymphoma with CD30 expression and anaplastic morphology exist, but all previously described cases retain expression of one or more B cell lineage-specific markers11, 12.
