Dear Editor, IgG4-related disease (IgG4-RD) is a systemic inflammatory disease that frequently affects salivary glands. Clinical features include elevated serum IgG4 levels and swelling of affected organs. Glucocorticoid remains a first-line treatment and generally shows a good therapeutic response. However, because the disease affects the elderly with comorbidities, glucocorticoid therapy is highly problematic in its side effects and complications from long-term use. Also, approximately half of the cases develop disease relapse during glucocorticoid tapering. Therefore, new therapeutic strategies with efficacy and fewer side effects that can replace glucocorticoid are apparently desirable. So far, rituximab or dupilumab has been reported to be effective in monotherapy in this disease [1]. We herein report the first case in which tocilizumab monotherapy, an antiinterleukin (IL)-6 receptor antibody, was successful against IgG4-RD.
