SIR, Iti a recent article Crow (1981) states that patients with allergic sensitization to formaldehyde-releasing bioeides (FRBs) have negative patch tests to formaldehyde. This assertion is incorrect. DiJngemann, Borelli & Reber (1964) reported positive patch tests to I" „ formaldehyde in five of sixty-three workers who were allergic to one or other of two FRBs (Grotan BK or Dibaktolan II). Roed-Petersen (1977) found positive patch tests to formaldehyde (concentraiioti not slated) in seven of thirteen patients with positive reactions to Grotan BK 2",, aq. Hjorth (1979) described a patient with industrial dermatitis in whom a positive patch test to 2",, formaldehyde led to the suspicion of sensitization to an FRB. Subsequent patch tests to Parmetol K50, an FRB to which he was exposed, were positive and he was later found also to react to Forcide 78, another FRB (Hamann, 1980). Control patch tests in seventy-four patients with Parmetol K50 10",, pet. (Hjorth, 1979) produced delayed reactions in seven (observed on days 17-50). One of two patients who were further investigated gave a positive reaction to 2'\, formaldehyde to which he had previously not reacted. In a further sixty-six controls tested with Parmetol K50 2",, pet., only one was positive, and he also reacted to 2",. formaldehyde. I have recently reported eight patients with positive patch tests to Grotan BK (Dahl, t98i) and have since seen an additional case from the same factory. Patch tests to 2",, formaldehyde were positive in three of these nine workers. A substantial minority of patients allergic to FRBs show positive patch test reactions to formaldehyde. For Grotan BK the figures were 33",, in my own series and 54",, in that of Roed-Petersen (1977). The finding of an unexplained reaction to formaldehyde in the standard battery may lead to the detection of hitherto unsuspected allergy to an FRB (Hjorth, 1979). Although these agents are extensively used as biocides in soluble coolant oils, similar compounds may be found in cosmetics and hair-shampoos (Rycroft, 1978; Hjorth, 1979) thus offering other potential sources of exposure and sensitization.
