Twenty-four carefully selected cases of primary malignant tumors of nerves are reviewed. These tumors were selected on the basis of being confined to large, anatomically identified nerve trunks in the absence of multiple neurofibromatosis. All malignant neurilemomas were highly cellular and presented predominantly a pattern of interlacing bundles with easily found mitotic figures. The only feature distinguishing a malignant neurilemoma from a fibrosarcoma is origin within the nerve trunk. The reticulin stains were not helpful in differentiating these 2 entities. Four of 15 traced patients have survived more than 5 years since the last surgical treatment. Twelve of 19 patients subjected to simple or radical local form of surgical treatment have experienced a recurrence of the tumor. It is suggested that primary amputation may give the patient the best chance of cure. When this procedure is not anatomically feasible, radical local excision should be attempted.
