MethodsThe communities. Tecumseh is a city with a population of 7,500 located in southeastern Michigan. The city and surrounding semirural area, with a total population of 10,000, has been under study since 1959 in a longitudinal investigation of chronic diseases and, since 1965, of acute infections [6]. For the surveillance of respiratory diseases, a panel of families is contacted weekly by trained lay interviewers to determine if there have been any new illnesses. Specimens for microbial isolation are collected if the illness is reported within 2 days of onset. In the weeks following the initial report, specificfollow-up questions are asked to determine the course of the illness. The families are kept under surveillance for 1 year and are then replaced by others. During the fall and winter of 1968-1969, 60% of the families were drawn randomly from all families in the community with children, whose adult members were under 45. The remainder were families in which 1 member had a chronic respiratory or cardiovascular disease; also included were matched comparison or control families.
