Magnetic resonance imaging screening for transient tremor incidentally detected an intracranial aneurysm and right internal carotid artery occlusion in a 62-year-old woman. Digital subtraction angiography revealed an abnormal vascular network in the cavernous portion of bilateral internal carotid arteries with a saccular aneurysm with a 10-mm maximum diameter in the left cavernous portion (Figure 1A and 1B); therefore, she was diagnosed with carotid rete mirabile. 1 The 4 dimensionalflow magnetic resonance imaging2 revealed an aneurysm within the carotid rete mirabile network (asterisk) with 2 inflows from the lateral aneurysmal wall, a sub-inflow from the aneurysmal neck, and one outflow (Figure 2A; Video).The main inflow making a whirl stream inside the aneurysm was observed (Figure 2B). Inflow is upward alongside the aneurysmal wall (Figure 2C), whereas the outflow is downward at the aneurysmal center (Figure 2D). Based on the flow dynamics on 4 dimensional-flow magnetic resonance imaging, our patient was followed conservatively.
