In this study, the accuracy of the Alvarado score in diagnosing acute appendicitis in children and adolescents was evaluated among patients seen at the emergency unit of the Instituto Materno infantil de Pernambuco, in Pernambuco, Brazil. A diagnostic test study, using clinical and laboratory scores to diagnose acute appendicitis, was carried out, with histopathology as the gold standard. A total of 81 children and adolescents with symptoms and signs consistent with acute appendicitis were examined during March-October 2002. With a score of ≥5 points set as the cut-off point, the sensitivity, specificity, positive predictive value, and negative predictive value of the Alvarado scoring system were 92.6%, 63.6%, 86.2%, and 77.8%, respectively. Thus, the Alvarado score can be a valuable tool in screening children and adolescents suspected of having acute appendicitis. It is less invasive, simple, fast and easily reproducible.
