The Neuropathologic Registry of Pediatric AIDS (Registry) was established in 1990 to collect and analyze data from cases from several medical centers, each with only a limited number of pediatric AIDS cases. The Registry's database includes detailed neuropathological data, general clinical data (risk factors, gestation, symptoms), and autopsy data (general findings, specific organ abnormalities, infections) of children up to age 13 that died of AIDS. All data are confidential. The database does not contain names, addresses, or identifying numbers. The only number is a linking laboratory number assigned by the referring physician. The Registry currently contains data from 156 pediatric AIDS cases. The most common mode of transmission of HIV was maternal-fetal, with only 13 transfusion-related cases, including two hemophiliacs. In only six of 156 cases was the central nervous system normal grossly and histologically. In the remaining 150 cases (96. I%), the central nervous system showed abnormalities, with several lesions occurring in combination in many of these cases. Microencephaly/brain atrophy was seen in 99 cases (63.5%), and a brain-weight deficit of 5% or more was seen in 83 out of 113 cases for which the brain weight was reported. HIV encephalitis was seen in 60 (38.5%) cases.
