Modern clinical and basic scientific research has changed our paradigms of the progression of periodontal disease. Many recent studies have been designed to improve periodontal diagnostic processes via host-based tests that utilize current understanding of the progression and pathophysiology of the disease process (38). The long-term aim is that treatment and prevention of periodontal disease will be founded on diagnostic tests based on etiopathogenic factors rather than just clinical experience. Diagnostic test kits based on matrix metalloproteinases or their inhibitors may be crucial in this respect. This chapter discusses the importance of the matrix metalloproteinase family in the pathogenesis of periodontal disease and focuses on likely candidates for matrix metalloproteinase and tissue inhibitor of matrix metalloproteinase tests for which there is clinical data indicating their potential utility.
