In 1989, we had the opportunity to present a paper entitled “Microbial risk factors for destructive periodontal diseases”(370) at a conference on risk assessment held at Chapel Hill, North Carolina. This chapter is based in part on that presentation. However, since a surprisingly large amount of new information has been published in the last 4 years, the chapter will be considerably extended both in terms of data and viewpoint.Destructive periodontal diseases may be thought of as a series of infections that affect individual or multiple periodontal sites within the oral cavity. These infections have a number of properties that make them difficult to study (as discussed below), not the least of which is the fact that flare-ups may be caused by a series of different microbial species and take place at various affected local sites at irregular time intervals. This chapter 1) summarizes some of the difficulties encountered in seeking etiological agents of destructive periodontal diseases, 2) indicates the currently suspected periodontal pathogens, 3) examines the risk of periodontal attachment loss associated with the presence of specific bacterial species, 4) examines the elements of the host parasite ecology that may affect disease progression and 5) suggests a possible coupling of microbial diagnostic tests with the selection of periodontal therapy.
