A measurement-oriented prosthetic management program is described, and treatment results from 36 patients are presented. Sequential measurements of “nasalance” were used to help define the pretreatment speech status of each patient, to identify specific results obtained, to modify the prosthesis by use of definitive information, and to help isolate and overcome speech problems met in assisting the patient to gain full benefit from the prosthesis. Differences among patients with different types of problems are cited, and an attempt is made to account for some of the variations in the results obtained. Specific attention is given to regression in “nasalance” following the initial use of prostheses by patients with congenital palatopharyngeal insufficiencies.
