Oxygenated derivatives of arachidonic acid (termed eicosanoids) and platelet-activating factor (PAF) are lipid mediators produced by cells involved in the mediation of pulmonary inflammation. These compounds have potent biologic activities relevant to the pathogenesis of these inflammatory reactions. This review will focus on the mechanisms of action and pharmacologic effects of these lipid mediators, their cell sources, and the waysin which these products lead to inflammation and airway injury. Specific products to be discussed are leukotrienes (LT), prostaglandins (PG), thromboxane (Tx), and PAP.
