A 65-year-old man with no specific medical history was admitted. Laboratory data revealed rapidly worsening renal function of serum creatinine of 2.6 mg/dL. Anti-neutrophil cytoplasmic antibodies (ANCAs) were elevated (MPOANCA 152.5 U/mL and PR3-ANCA 43.8 U/mL). Clinical, serological, and histopathological findings met the diagnostic criteria for granulomatosis with polyangiitis (Fig. 1). Interestingly, MGCs containing 4 to 7 nuclei situated in peripheral areas of the cytoplasm were detected in two glomerular crescents, which were positive for CD68 (Figs. 2, 3).
