DISCUSSIONThe most important finding in this case report is that histologic evidence of an inflammation of the vestibular nerve was obtained. A diagnosis of vestibulocochlear neuritis is sustained because the cochlear nerve was also found to have an inflammatory infiltrate, although it was not as extensive as the vestibular involvement. The first conclusion to be drawn is that inflammation of the eighth cranial nerve does occur. A viral infection has been implicated in the cause of VN by most papers. 2-4 Most authors favor a viral theory with VN on the basis of a preceding viral infection in 40% of patients, 1 on the similarity to a reported case of herpes zoster oticus, 3 and also on the reported increased incidence of VN in viral epidemics? These proponents of a viral theory do not have histologic evidence to support their claim. A thorough histopathologic assessment of the specimens in this article revealed no evidence for a viral cause. The histologic findings were consistent with chronic inflammation, and there was nothing to suggest either a vasculitic or granulomatous cause. This in-depth analysis of the diseased nerve itself
