Trauma remains a leading cause of mortality in all age groups, with an increasing global impact as the population ages. 1 Advances in pre-hospital care, damage control surgery, and blood-based damage control resuscitation have led to significant improvement in initial survival from exsanguination after serious injury, 2 but these patients often remain critically ill and are at risk for ongoing sequelae, including organ failure, 3 development of chronic critical illness, 4 and late mortality after injury. 5, 6 Despite intensive ongoing efforts, many aspects of postresuscitation physiology and metabolic function, and, more importantly, potential opportunities to improve outcomes, remain incompletely understood. This study by Cyr and colleagues 7 is timely and enlightening. The authors performed a large-scale comprehensive metabolomic analysis on serum from injured patients, then analyzed the metabolome for changes over time, prioritizing metabolites for further scrutiny using a consensus approach with rank ordering. Almost half of the metabolites examined consisted of lipids. The authors found specific changes in the pattern of sphingolipid expression over time, with increased sphinganine and sphingosine species early after trauma, followed by enrichment of ceramides in the later timepoints. They also found sphingomyelin enrichment at each time point. To take their analysis a step further, the authors evaluated alterations of sphingolipid species at days 2 through 5. They demonstrate that specific sphingolipid response patterns are associated with circulating cytokine levels as well as improved outcomes after blunt trauma.Sphingosine was initially discovered in the 1880s and named after the Sphinx, an enigmatic mythological creature that is part human, falcon, cat, eagle, and lion. 8 We now understand that sphingosine is a member of a large family of sphingolipids, which are characterized by a common sphingoid backbone, with the addition of different head and tail groups, leading to hundreds of individual classes of sphingolipids, each with distinct structures and functions. 9 Ceramide, composed of sphingosine with the addition of a fatty acid, is central to all sphingolipid pathways and represents an important family of
