New research published in Nature Metabolism shows that exosomes secreted from hepatocytes have different effects depending on the stage of obesity. In early obesity (mice fed a high-fat diet for 4 weeks), the exosomes were shown to enhance insulin efficiency. The authors found that this beneficial effect was mediated by miR-3075, which was enriched in the exosomes. By contrast, in mice with chronic obesity (fed a high-fat diet for 16–18 weeks), the exosomes promoted proinflammatory activation of macrophages, which resulted in insulin resistance. These exosomes had high levels of miR-434-3p, which promotes polarization of macrophages towards an M1-like state. The authors suggest that miR-3075 could have insulin-sensitizing effects in humans, but this needs to be investigated further. In addition, targeting miR-434-3p could prove to be beneficial.
