The most successful thrombolytic enzymes are those which act by producing plasminogen activation. Much of the current knowledge has been gained by the use of streptokinase and therefore the preceding description is largely concerned with this activator. It may be, however, that the next few years will see urokinase replacing streptokinase as the enzyme of choice. Biochemical studies following infusion of activator have extended understanding of the plasminogen-plasmin system, and thrombolytic therapy, despite recognized difficulties, has shown considerable promise. It is appreciated that this is not the final answer to the problems of thrombosis, which must await a truly etiological approach.
