Prolonged Th-2 cell activation and increased viral replication in HIV-Leishmania co-infected patients despite treatment

In a pilot study, serial immunological and virological investigations were made in 8 patients in Italy with visceral leishmaniasis (VL). Three of the 4 patients who were HIV-positive developed AIDS within 8 months from the onset of acute VL. Serum antileishmanial antibodies were present in all subjects and all patients received a single course of meglumine antimonate. Treatment was successful in all patients. In those patients co-infected with HIV andLeishmaniapersistently elevated Th2cytokine levels were demonstrated in serum, together with declining concentrations of interleukin (IL) 2 and IL 12. There were also significant increases in serum levels of interferon γ. Serum HIV RNA concentrations also increased during follow-up. It is concluded that in co-infected patients these data seem to indicate an irreversible switch from type 1 to a type 2 response, in spite of therapy. It is suggested that this phenomenon may favour the progression to AIDS.
