Two of the criticisms of the SUPPORT trial are closely related: first that there was no “usual care” group, which should have been included to make the trial informative, and second that because being randomized to a protocol deprives a research subject of receiving usual care, that is, the clinical judgment of the physician, this should have been stated in the consent form. These criticisms are based on a misunderstanding of neonatal intensive care.
