To the Editor: Over the past decade, physicians across the United States have described an acute amnestic syndrome distinguished by bilateral hippocampal abnormalities on brain imaging in the setting of opioid misuse, particularly involving fentanyl. Patients with this rare opioid-associated amnestic syndrome (OAS) demonstrate marked anterograde and milder retrograde memory loss for weeks to months or longer. At times, OAS is initially observed after tracheal extubation after an overdose, but often occurs without a known concomitant event. 1–3 Through the stimulation of mu receptors, fentanyl and other opioids are thought to induce hippocampal hyperexcitability by inhibition of inhibitory interneurons, a possible mechanism contributing to OAS. 4 However, the dearth of OAS cases associated with fentanyl use in clinical settings has been raised as an area of uncertainty as to the exact role of opioids in the pathophysiology of this syndrome. 5, 6Although documenting the frequency of a rare condition attributable to clinical use poses a unique challenge, the US Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) offers a surrogate method to estimate the occurrence of this phenomenon.
