本文报告两例肝内胆管结石合并肝总管狭窄的病例。动脉压迫胆管的可能诊断是通过将直接胆管造影片叠加在血管造影片上获得的。最后的诊断是在手术中。第1例患者通过右肝动脉前方的肝总管切除和肝总管空肠Roux-en-Y吻合术成功治疗。第2例患者行外侧段切除术和胆管空肠吻合术。
Two cases [human] of intrahepatic gallstones with stenosis of the common hepatic duct caused by overriding of the right hepatic arterty are described. The probable diagnosis of the compression of the bile duct by the artery was obtained by superimposing a direct cholangiogram on an angiogram. The final diagnosis was made at operation. The 1st patient was successfully treated by resection of the compressed common hepatic duct with Roux-en-Y hepaticojejunostomy anterior to the right hepatic artery. In the 2nd patient, lateral segmentectomy and cholangiojejunostomy were performed.
