We found systolic blood pressure (SBP)> _130 mmHg at age 50 to be associated with a 1.45 times great hazard ratio (HR) of dementia. 1 We estimated associations when hypertension was defined in three ways:> _140 mmHg,> _130 mmHg, and> _120 mmHg with HRs of 1.39 (1.08, 1.80), 1.45 (1.18, 1.79), and 1.20 (0.97, 1.48) in analyses adjusted for sociodemographic measures. We concluded that SBP is associated with increased risk of dementia below the conventional> _140 mmHg threshold. Our findings are supported by preliminary results from the SPRINT Memory and Cognition IN Decreased Hypertension (SPRINT MIND) trial. 2 The combined outcome of mild cognitive impairment and all-cause dementia in this trial was reduced by 15% in the intensive treatment group (SBP target< 120 mmHg) compared to the standard treatment group (SBP target< 140 mmHg).Gregson and colleagues 3 analysed data from the Clinical Practice Research Datalink (CPRD) and report their results do not support our findings. We could not work out how they arrived at this conclusion as they analysed only the HR associated with SBP> _ 140 mmHg, not the risk associated with the 130mmHg and 120mmHg thresholds. They then examined event rates (Figure 1) using a somewhat different categorization of SBP (< 115, 115–124, 125–134, 135–144, 145–154, and> 155) and without undertaking a formal comparison of difference in event rates concluded that rates in all categories were similar.
