A 14-year-old girl was transferred to the ER because of fever and disturbance of consciousness. At 1 month of age, she had been diagnosed as having tuberous sclerosis complex (TSC) and had thereafter visited the hospital regularly because of mild mental retardation and epilepsy. Two years previously, she had undergone surgery for traumatic cardiac tamponade. At admission, we suspected sepsis and therefore performed imaging tests. A chest X-ray revealed more than 1 ‘needle-like’foreign body in the chest and another in the abdomen (Fig. 1, Panel a). Transthoracic echocardiography and computed tomography showed that the foreign bodies penetrated the anterior wall of the right ventricle and ventricular septum and reached the posterior wall of the left ventricle (Fig. 1, Panel b–d).Emergency open-heart surgery under cardiopulmonary bypass was performed. In total, 6 sewing needles were removed from the anterior wall and outflow tract of the right ventricle, the ventricular septum, and the left ventricular wall. Some of the needles were rusty and with vegetation attached (Fig. 1, Panel e). We also performed a mitral valve
