Bisphosphonate treatment has been used in infants from 2 months of age (Plotkin et al 2000). Although we were alert for complications, symptomatic hypocalcaemia still occurred. This indicates that infants in their first month of life have fragile calcium balance. Therefore, we suggest that intravenous calcium supplements may be necessary for neonates receiving bisphosphonate treatment, especially when oral intake is not secured.
