Two black Haitian infants who died with Kaposi's sarcoma and immunological evidence of acquired immune deficiency syndrome (AIDS) are described. One presented with diarrhoea, oral candidiasis and seborrhoeic dermatitis with subsequent development of recurrent otitis media, lymphadenopathy and pulmonary infiltrates. T4/T8 cell ratios dropped from 1.7 at 4 months of age of 0.32 at 6 months. Circulating immune complexes were present and immunoglobulin levels reduced. A 22-month-old brother of this male infant also had pulmonary infiltrates, recurrent infections and a T4/T8 ratio of 0.3. The child died at the age of 7 months and herpes simplex virus was recovered from the lung.The second infant presented at 4 months of age with oral candidiasis, atopic dermatitis and otitis media. At 8 months he was admitted with hepatosplenomegaly, fever and acute respiratory distress. Lung biopsy revealedPneumocystis carinii.T4/T8 ratio was 1.69. Serum IgG levels were low. The child died of respiratory failure. Both infants showed lymph node splenic evidence of Kaposi's sarcoma of mixed cellular type. In one the thymus was also involved.newline˜K.C. Watson
