The precise mechanisms involved in the etiopathogenesis of type I or insulin-dependent diabetes mellitus (IDDM) remain uncertain but are the subject of much current investigation. A popular current formulation includes genetically determined diatheses that predispose the individual to IDDM (diabetogenic genes), islet cell damage triggered by environmental factors (viral infections andlor chemical toxins), and immunologically mediated islet beta-cell destruction. The genetic diathesis involves the HLA system, as has been reviewed elsewhere in this Yet, it remains uncertain as to whether diabetogenic genes exist either within the HLA region or in close proximity thereto or whether there is rather a quantitative susceptibility conferred by particular HLA-related genes, as suggested by Nerup et al. I Whether environmental events are important in the pathogenetic sequence is unresolved. There is compelling evidence that they may initiate the pathogenetic processes in genetically predisposed animal^,^ but the evidence for their role in IDDM in human beings is less secure. Although it has been argued that the substantial discordance of IDDM in
