In the current day, we must limit work hours, and to some extent, I believe it is a good and right thing to do. It will be interesting to see how restriction of work hours affects quality of life as measured by psychological testing, a project we are working on in our department currently. However, let us not be deceived into thinking that time away from clinical educational opportunities necessarily translates into improved quality of life. Quality of life is being able to do what one wants to do, needs to do, is called to do. For some, that may not include spending time with their mother-in-law.Finally, the authors conclude that the increase in US medical student filled categorical first-year positions in general surgery is due to an increased interest in general surgery, possibly due to work hour reforms. A more reliable indicator of interest (that I have been unable to obtain from the National Resident Matching Program) would be the number of US medical students applying in general surgery over this time period. Match rates are related not just to medical student interest, but in the programs’ interest in matching US graduates. I appreciate your publishing this interesting and provocative article.
