Traditionally, the clinical syndrome of heart failure has been viewed as a pathophysiologic state in which ventricular performance is depressed and leads to symptoms related to low cardiac output and elevated left ventricular filling pressure. It would seem logical to assess the patients with heart failure by hemodynamic evaluation, however, hemodynamic measures are not always abnormal in the failing heart. More recently, heart failure has been defined as a syndrome in which cardiac dysfunction is associated with reduced exercise tolerance, a high incidence of ventricular arrhythmias and shortened life expectancy.
