MethodsSix normal volunteers (5 male, 1 female ages 25-34) were studied (protocol approved by the Human Investigation Review Committee, New England Medical Center). On day 1 of the study, blood was sampled for TSH and Cortisol every 30 minutes for 24 hours. On day 2 a standard insulin tolerance test was performed. Blood was sampled for baseline serum TSH and Cortisol determinations (time 0) and regular insulin (0.1 units per kg BWT) was injected IV. Blood was then withdrawn at the following times for serum glucose, TSH and Cortisol: time 15', 30', 45', 60', 90', 120', and 240 minutes. Symptomatic hypoglycemia developed in all subjects following insulin administration (glucose< 45 mg%). The serum was stored frozen (—20 C) prior to assay for Cortisol and TSH. An immunoradiometric assay developed by Serono Diagnostics, Randolf, MA was used to determine TSH levels (sensitivity 0.1-0.2 ulU/ml; additional details on the assay precision have been previously published by Cobb, Lamberton and Jackson (1984). Serum Cortisol was assayed by a competitive protein binding technique. All samples for each subject were run in duplicate in the same TSH and Cortisol assays. In order to determine the effect of insulin induced hypoglycemia on TSH we calculated an index of TSH secretion, the integrated TSH response. This index, expressed as uUTSH/min/ml, is based on the integrated area between the baseline (time 0) TSH concentration and the subsequent TSH concentrations following insulin. The results were analyzed with the paired student's t-test (two-tailed) and significance was defined at the P< 0.05 level. Correlations between serum Cortisol and TSH were performed using a linear regression equation (Y= A+ BX)(CLINFO software program).
