This landmark study of p16INK4a delineation of cervical epithelial neoplasia is a major advance; its surrogate validity is convincing. 1With this in mind, revision of criteria for the lower threshold of interpretation may be useful. Figures 1C and D show focal positivity (designated as negative) and may still have surrogate value; Figure 4B and, to a lesser extent, Figure 3B (designated as positive) also have focal areas of positivity adjoining denser areas of immunohistochemical staining. Focal positivity may represent evolving genetic instability in all four illustrations. Assuming that valid false-positive staining is excluded, even slight positivity may therefore merit a positive reading. Such a modification may more sharply define reactive versus CIN 1 groups, the major thrust of this admirable work. It may streamline followup studies by eliminating the “unclear” category, by improving the concordance of “normal”(94%), and by bringing the concordance of CIN 1 (91%) closer to the concordance level of CIN 2 (100%).
