The article presents a case study of an infant with Down syndrome and Tetralogy of Fallot (TOF) who experienced paradoxical spells during adrenocorticotropic hormone (ACTH) treatment for West syndrome. It is reported that the patient, with mild right ventricular outflow tract obstruction, developed anoxic spells, bradycardia, and cyanosis during ACTH therapy. It is further reported that the spells ceased after terminating ACTH, and subsequent treatment with topiramate controlled spasms.
