Testing with flawed multiple-choice questions (MCQs) yields a failure rate up to 25% greater than testing with well-constructed MCQs (3). This disparity, along with the quotations above, testifies to the need for a concise guide to creating effective MCQs. In radiology, burgeoning demand for skill in MCQ writing stems from mandated self-assessment in continuing medical education (CME), expanding participation in maintenance-of-certification programs, and implementation of new examination formats by the American Board of Radiology. However, the need for better MCQs is not limited to our specialty: In a 2006 review of 40 CME questions from the New England Journal of Medicine, three or more violations of proper item-writing tenets were found in every question (4). This brief guide to creating good MCQs provides practical tips distilled from more detailed and comprehensive articles by other authors (5–11) (Figure).
