Gastroent.1974,9, 511–518.In 9 patients with duodenal ulcer graded doses of pentagastrin (0.15, 1.5, or 15µg/kg/hr) were infused intravenously on separate days in random order. The tests were performed before, 3 and 6 months after proximal gastric vagotomy (PGV). At 6 months one or twoµg/kg/hr of carbacholine was infused simultaneously with pentagastrin. After PGV maximal secretory capacity of acid and pepsin was reduced by 50 % and 42 % respectively. The sensitivity of the parietal and chief cells to pentagastrin stimulation decreased. Carbacholine restored the sensitivity of the parietal cells to pentagastrin to preoperative level but did not influence the mean maximal secretory capacity of acid. Carbacholine influenced the postoperative mean maximal output of pepsin without changing the sensitivity of the chief cells to pentagastrin. In 2 out of 9 patients, howewer, carbacholine nearly restored the prevagotomy level of pentagastrin-stimulated acid and pepsin secretion.
