Thirty-three patients with acute leukemia underwent chemotherapy while in protected environment (PE) units and receiving prophylactic antibiotic regimens. The effects of chemotherapy in these patients were compared to two groups of selected control patients. The duration of remission and survival was significantly longer for patients treated in PE units than for the combined groups of control patients. The median duration of complete remission was 55 weeks and 26 weeks, respectively. The patients in the PE group received more Intensive chemotherapy and had more severe and prolonged neutropenia. The frequency of infection was significantly less among patients in the PE group when related to the level of circulating neutrophils. The complete remission rate was slightly higher for patients treated in the PE units than for either control group (61 vs 55 vs 42 per cent), and more of the PE patients survived long enough to receive an adequate trial of chemotherapy, but these differences were not statistically significant
