METHODSGlomeruli were isolated from the superficial renal cortex of normal male Sprague-Dawley rats. Isolation medium was isotonic and contained 4 g/dl of bovine serum albumin. Aliquots of glomeruli were incubated in control medium or in medium containing either CsA, 10-6 M (Sandoz, East Hanover, NJ), or vehicle (cremophore, 10-6 M; Sigma Chemical Co., St. Louis, MO) for 30 min at 37 C. Following incubation, glomeruli were washed twice with medium without CsA or cremophore. Washed glomeruli were then incubated in medium containing 2% v/v serum from either the FSGS patient or pooled normal serum for an additional 15 min at 37 C. Individual glomeruli were selected for video-microscopic observation and the initial medium was replaced with lower oncotic pressure medium containing 1 g/dl bovine serum albumin. This exchange of medium resulted in the influx of fluid into the capillaries, resulting in an increase in glomerular size. The change in glomerular volume, ΔV, in response to an applied oncotic pressure was calculated from the change in glomerular size. The rationale and details of the calculations of albumin reflection coefficient are published elsewhere (8). In short, albumin reflection coefficient, σ albumin, was calculated as: ΔV experimental/ΔV control. Albumin permeability, P albumin, was calculated as 1-σ albumin (8). A total of 13-15 glomeruli from 3 rats were studied in each experimental condition. Average P albumin was calculated for each condition and differences were assessed using Student's t test. Significance among various groups was expressed asP< 0.05.RESULTSAs shown in Figure 1, FSGS serum caused a significant increase in P albumin of glomeruli as compared with normal pooled human serum (0.71±0.04, n= 13, vs. 0.01±0.05, n= 15, P< 0.001). Preincubation of glomeruli with CsA prevented this increase in P albumin by FSGS serum to an average of 0.09±0.01 (n= 15), which is not different from control (P albumin 0.01±0.05, n= 15). On the other hand, preincubation of glomeruli with vehicle alone (cremophore) had no protective effect on an increase in P albumin by FSGS serum (0.67±0.06, n= 15).DISCUSSIONThe recurrence of FSGS is a common and difficult clinical problem that results in morbidity from proteinuria and nephrotic syndrome and loss of renal function in some patients. The mechanism for recurrence has not been well defined, but the presence of a circulating factor has been inferred. The favorable therapeutic effect of plasmapheresis or treatment using protein A columns supports this thesis (3, 4, 9). It has also been reported that injection of plasma fractions recovered from the fraction bound to protein A columns can cause proteinuria in rats (9). We developed an in vitro assay to measure the increase in P albumin of glomeruli (8) and have used this method extensively to study the effect of various experimental treatments on glomerular albumin permeability. We have documented that incubation with media containing sera of patients with recurrent FSGS can cause an increase in P albumin within 10 min of incubation (4), whereas normal pooled human serum, sera of patients with nonimmune renal diseases, and sera from other transplant recipients have no effect on glomerular P albumin. This increase in P albumin by FSGS sera is comparable to the effect seen by anti-Fx1a antibody (10), superoxide (11), and hydroxyl ion (12). We have also found that the presence of a high level of serum activity, defined as P albumin> 0.5, prior to transplantation is strongly associated with subsequent recurrence of FSGS (8).
