In conclusion, Koster et al. have persuasively argued that small numbers of transfusions of leucocyte-depleted PRBCs do not increase short-term mortality or morbidity, adding to a growing amount of data supporting the short-term safety of minimal transfusion. Whether they have identified the transfusion inflection point (2 units of PRBCs) for long-term benefit will require further prospective study. Although further questions remain, we congratulate the authors on an excellent clinical series.
