International travel plays a major role in the global spread of antimicrobial resistance (AMR). 1 Travellers to endemic areas return to their home country frequently colonized with multidrug-resistant organisms (MDROs) and can introduce them into their households, communities and healthcare facilities. 2 Outbreaks of MDROs imported from Southwest Asian countries have increasingly been reported in Japan. 3 Because MDROs are extremely expensive and difficult to control once acquired, risk assessments of MDROs colonization in patients with respective travel histories and control measures based on the epidemiology of both the home and destination countries are crucial to prevent further spread of MDROs if the patients are admitted to hospital. 1We experienced a case of post-neurosurgical meningitis due to extensively drug-resistant Pseudomonas aeruginosa (XDRPA) imported through medical evacuation (medevac) to a university hospital in Japan. The patient underwent craniotomy in a Southeast Asian country 3 years ago and presumably acquired XDR-PA at that time. We examined the XDR-PA colonization of the patient and the environmental contamination of the patient’s room by whole-genome sequencing (WGS)
